The "Ask The Doctor" site feature gives you access to a fillable Quick Quote form. Click the link below, select the relevant impairment, complete the fillable form, and click submit. An email including the detail you provide is then sent to the CPS Underwriting Department. Our participating companies will have the opportunity of evaluating the information and making a tentative offer for life insurance for the client. No signatures, no application, no time consuming investigations or expensive Attending Physicians' Statements - a complete evaluation in less than 48 hours. All questions must be completed.

The Informal Inquiry Form or Informal Inquiry 15-Point Type (required in CA) needs to be completed, signed and submitted to CPS when APS(s) need to be acquired prior to shopping an impaired risk case.
- For cases with face amounts of $1 million and/or $5,000 estimated annualized premium and up, CPS will obtain the medical records upon receipt of the Informal Inquiry Form.
- For cases with face amounts of $250,000 and/or estimated annualized premium of $1000 and up, please submit the medical records along with the Informal Inquiry Form.

On occasion, a carrier will require that a questionnaire be completed during the underwriting of a case. THESE ARE NOT THE SAME AS QUICK QUOTE QUESTIONNAIRES.
